Your son has PANDAS from the strep. You need to get more info about PANDAS at www.pandasnetwork.org. Most kids can revocer w/ a longer term of antibiotic and then may have to stay on a prophilactic dose (once per week thru puberty). Azithromycin works well but others have success w/ augmentin which can be tough on the gut (nausea/vomiting). If the longer term of anitibiotics do not clear the symptoms (OCD, meltdown etc). Then a short steroid burst often helps. THe last treament resort treatment option is IVIG. PANDAS can be very debilatating so the sooner you aggressively treat it the easier it is. My son sufferred from PANDAS for years before we figured it out. He is now being treated w/ IVIG/steroids/antibiotics and finally recovering. Which state are you in? I may be

able to help you find a good PANDAS doc. There are very few docs in the country that are knowledgable.....most don't believe in it.

I would look into PANDAS, and try to find a provider who knows what it is. It is controversial, but there are many specialists who absolutely believe in it, and are researching and treating it. It sounds like your child is definitely suspect. My son is 10 with AS, and he is a suspected PANDAS kid...although mine had his tonsils removed at age 4. Even so, when he gets strep, it takes rounds of antibiotics to get rid of it, and then it throws his gut off, and we have major behavior deterioration. He was just dx'd last week with strep (again) and we are doing double probiotics, ibuprofen, along with the Keflex. I am doubtful the 10 day course of Keflex will do the trick, but we will see, and we are keeping a close eye on behavior, trying to support him as much as possible and keep anxiety to a minimum. In fact, I joined this group because someone on my AS list recommended it as a resource to get ideas on treating PANDAS.

I also would definitely get rid of the tonsils. Believe me, I had a pediatrician who refused to get rid of mine, and I was miserable with nasty infections most of my life. When I went to college, I went to an ENT on my own and asked to have them removed. He took one look and said "Why on earth do you still have them!!" Good luck!

Do all strep infections can cause a child to become a pandas kid or only a certain kind of strep?2. I am falling seemingly often w throat infections, last time my ASD had it, I am sure l passed it to him. yes what you discribed does NOT sound like a PANDAS kid. a PANDAS kid has a whole shift in personality, mood, behavoiral, responces to life, to achedmics, with OCD, ODD ect. sounds like you are no there.

as far as question number one a lot isnt known. from the research I have done I have seen families with PANDAS regress with probiotics with strep lines in them, seen constant markers for stool GAMMA, but the only pathogen currently studied is regular strep as what you think as in strep throat, but those living with PANDAS see a much broader range of things setting it off

present, should investigate more for the probability of PANDAS?

honestly neither. gamma strep is what I have found in ALL cases of PANDAS thus far with out exception. it shows on stool tests. it's not about the type of strep anyone. it's about the symptoms. if you see SYMPTOMS you THEN investigate for strep

You can learn more about PANDAS at www.pandasnetwork.org The symptoms are OCD, rages, anxiety, separation anxiety, repetitive behaviors, washing hands over and over, deterioration of handwriting and math skills, etc. The symptoms set in following a strep infection. THere is generally a sudden onset of symptoms but it can also be gradual. With each exposure to strep the symptoms can get worse and worse.

You can do a blood test and check ASO/DNASE titers but 5-% of PANDAS kids do not have high titers. When my son had mild PANDAS his titers were high and when he had severe PANDAS symptoms his titers were low. You need to diagnose based on the symptoms.

Any strep infection can cause PANDAS but not all people who get strep get PANDAS. Generally people who have immune dysfuction have a higher rate of getting PANDAS than healthy people but perfectly healthy people can get PANDAS following strep.

The first line of treatment is antibiotic (azithromycin or augmentin). If that does not work then antibiotics w/ a steroid burst. Last resort treatment is IVIG.

You can give it daily for at least a year and see if that helps. I have taken

1000mg daily for over a year. I would think do whatever worked long term.

Which haemolytic? Gamma haemolytic is a bad one. My PANDAS kid is +4 for this.

Isa,Dr. MC developed a test (testing Cam-Kinase and anti-neuronal antibodies) to help diagnose PANDAS. I just got a script from my son's pediatrician to test ASO/DNSAE. Our doc wanted to do the Cunningham test on our son in Dec but there was a big back log of tests so we were not able to do it at that time. Not sure we'll bother now since we are already doing IVIG.......it's $400 and we may wait till we are thinking of stopping our monthly IVIG to test.

My 8 year old has had recurrent strep throat for past several months. I took him to an ENT who said his tonsils are huge and the strep is probably buried in his tonsils so they need to come out. I would like to avoid surgery if possible. He's done 3 courses/5 days (high dosage) of Zithromax which cures it each time but it keeps coming back. It's definately affecting his brain because when it flares he has major meltdowns, aggression and facial tics. Would a longer, prophylactic dose of Zithromax cure the strep or would it only keep the symptoms at bay until stopped? Would an immunologist be a good idea? Does anyone have any other suggestions for treating strep? Thanks very much.

It is till a big back log of tests in the Cunningham lab.LiminOur doc wanted to do the Cunningham test on our son in Dec but there was a big back log of tests so we were not able to do it at that time. Not sure we'll bother now since we are already doing IVIG.......it's $400 and we may wait till we are thinking of stopping our monthly IVIG to test. While on antibiotics my son stays on diflucan and Custom Probiotics. No yeast issues.

Well, it could be gamma strep and therefore, PANDAS.

Hi Limin,

I am always using new probiotics and also herbs to fight yeast, worms, viruses,

etc. My son also takes diflucan or ketaconazole regularly.

yeah strep in the yogurt would be enough to set off a flare I would ask also if thye used nitrous oxide when putting him under as kids with MTHFR mutations can't do it with out segnificant plumeting in b12 that would have to be replaced quickly or will cause damages

My son has pandas and we had his tonisils out per Dr K, he developed severe motor and vocal tics (which he had never had before) two weeks after the surgery, our ENT said there was no connection but seems odd to me the timing. I already checked w the lab, the result is alpha haemolityc....
