We had United healthcare ppo at that time (3 yrs ago).. and the Doc

was out of network. Dr Stelnecki said because he wrote on the

prescription no substitutes they had to pay for it because they

dident have a network to go to in our area so I had to go out of

network. It was paid for under DME 100% with no out of pocket

exspense to us. We since dont have that Insurance.
