I'd say that, just for the heck of it, if you're still having milky discharge off of the premarin, you should get your prolactin level tested. Milk discharge can be caused by several things (increased prolactin level due to pituitary tumor or hypothyroidism, over-manipulation, some medication will cause it, breast tumor, among others I can't remember), and should definitely be checked out thoroughly because it just ain't normal. Specific symptoms of a prolactinoma may include (depending on your prolactin level, tumor size and your body's reaction to increased prolactin): breast discharge; irregular or non-existent periods; headaches; vision problems; increase in breast size; excessive tiredness; (for me) unclear thought processes. But the discharge and the change in periods are the main 2. Many of these things can also be indicative of a thyroid problem, so if a prolactin test comes back high, your doctor should send you for an MRI of your pituitary, to see if a tumor is locatable, and should also run a HUGE battery of blood tests, especially checking for thyroid function.
