Dr. Melmed doesn' t always remove implants en bloc . . . That's one of the reasons we don't recommend him as readily as we recommend other doctors. I've exchanged numerous messages with him about the importance of doing total capsulectomies, and get (what I consider) double-talk in return. He tells us that he always does en bloc with silicone gel implants . . . but that saline shells are often too thin to remove en bloc . . . he often has reasons why he didn't remove everything - after the fact. He usually removes drains after 24 hours. Some women have told us about having a great deal of pain and swelling after explant that could have been avoided it the drains were left in longer. Dr. Feng has a list of criteria that determine when a total capsulectomy is necessary. Any woman who is symtomatic should have a total capsulectomy! - preferably en bloc. Dr. Melmed our most outspoken plastic surgeon - A really good man as well as an excellent surgeon . . . Our problem with him is that he (apparently) doesn't take saline problems as seriously as we feel he should. If we felt confident that he did, we would be happy to recommend him wholeheartedly. Nevertheless, it sounds like you are doing very well! . . . You may have some fluid buildup - but that should be absorbed in time. . .
