I had my explant last August. I sent all my required paperwork to Dow and then found out about the MDL and asked my attorney to call them he said let's not worry about that right now, so I called myself and they were great on the phone they sent me the paperwork for the same amounts you mentioned and I haven't received anything yet. Don't freak out just take one step at at a time.
