SSI publications you can download on the SSI website and others. The evidentiary findings, listing of disabilities, source documentations listings,and other resources and forms are all on the internet and the website for free. That website should be you guiding light as that is what I used along with letters from my co-workers, families, and friends to document my changes. I also got a copy of my job description, the Oswalt disability scale, my pain diary, and addressed all the things I could no longer do. A few weeks after my husband filed he got a long (like maybe ten pages) form to fill out on daily activities. It almost seemed repetative, but we left no blank answers. My husband got it on the first try and within 90 days of applying. Which we were thrilled, it came just in time. We had no income! I could not work, because I had to take care of him. I am sure that you will get some papers to fill out, make sure to get them right back out and copy them. That way if there are any problems, you have a record. But I sat and went over mine for 3 days making sure every thing was right on it There was not enough room on it to list all the doctors so I picked the 3 that I was or had seen in the last year that I was sure they were the most for me. A lot of people say they don’t even check with the doctors but they check the 3 I listed and one I didn’t list. Dear and to the room I have a rather un-easy question to put out to the group. I have never tried nor will I try to apply for SSI I get mine though the Veterans Affairs, so here's the question. I have a really close friend who is a female that has a really bad back due to a car accident and applied for SSI in the state of New York because of it. She met a new boy friend and low and behold she got pregnant within a short period of time in meeting him and had the child while the claim was pending. Her Claim for SSI do to her bad back was denied in two parts; 1 - That if she capable of having sex with a bad back than its not that bad at all to work. 2 - If you can go though child birth than you don't have a bad back. My husband made about 6 attempts (only staying for a few months at a time at each job and this was over a one year period) and he pretty much showed them he attempted and can not do the job/jobs he applied for. It's a tough call on their part, they are trying to eliminate fraud. What she needs to do is appeal, appeal, appeal. Do not let her reapply, because she will have to start from square one. Bennie My husband downloaded the worksheets off the web, There were some in there asking about the things I could no longer do. I can't remember how many I filled out but there were several. By the time we got done there were 37 pages of worksheets. Some asked if I fixed meals and how many times a month I fixed them. It also went as far as having me tell what I fixed. So they were very detailed worksheets that I filled out. On the sleeping question I even put down how I even dream of being in pain and waking in pain. I also put down that I could no longer lay on my right side. I tried to put everything thing I could think of on those worksheets. So I was pretty detailed in describing how it affected my everyday life. My husband went over them and put in things that I hadn't thought of. He even went as far as downloading a c.d with his data base he built about my condition. That way if they needed to find something they could. The guy told him that was ok he didn't think he needed it. My husband is very anal about things so with his help one way or another I will get my disability.
