When someone agrees to get into your ambulance, they do so with the understanding that you will transport them to an appropriate medical facility. In as much, whenever possible, you should take them to a facility of their choice, however, what if you're the last ambulance available in your service area and transporting them to a hospital an hour away (quite possible in rush hour urban or rural settings) takes you away from other possible patients. What if you deem the transport priority to warrant transport to the nearest appropriate facility, i.e. a breech birth that you can do nothing about? This is a civil matter, not criminal. If the patient says they want to cease treatment and transportation and you refuse THEN you might possibly face criminal charges.
