I will answer this for you as I recommended it to Kris. It is the

National Pain Foundation website, you do not have to log in, give

your e-mail to download it either.

You just go to the National Pain Foundation click MY Pain, it is in the list

and you print it off and you do not have to keep it online as with any of the

others you find.

I also stated you can just type in pain diary or pain journal in google and

their are many. I sent one from Glaxco, they make one, and most medication

companies have samples.

It is NOT spam and this foundation has assisted legislature for the Patient Pain

Act and has been around for years.

I have good experience with them, they have never solicited nor did I

get spam, you can even look at their forums without signing on.

I sent Kris three diaries but she needs to convert them to rich text

format to print which I guess I didn't explain well since she can't

open them.

Kris, you can download adobe reader for free and read all the

pdf and adobe files you find on your internet search and print

them out for yourself when you find one you like.

Hope that further explains what I meant and assures you it is not spam. Sorry, I

confused you in trying to help.
