So sorry to hear you're leaving us Allan, we'll all miss you!!! you've been a

delight to have aboard with your inquisitiveness, humor and sincerity in trying

to understand our plight. Thank you for all your answers too, they were

greatly appreciated.

Like wise you've given us a bird eyes view of what it is from your perspective.

We can certainly understand how buzy you must be with your daily activities but

hope you'll drop a line once in a while to let us know how your project is going

or just to say 'Hi'.

Gals, just a reminder, if you haven't sent your surveys in to Allan, now is the

This is actually

a pilot study for a larger survey that we have talked about doing. A member

of the List made the suggestion of doing it as a minisurvey. The List is

looking at its own membership and getting information on the membership.All of

you become collaborators on the project. Thus in essence the group is

evaluating itself.

By sending me the questionnaire below, you are essentially becoming a

collaborator on this pilot study and are allowing us to use the info as we

see fit. If we ever use it, the information would of course be

unattributed.You are also essentially giving permission for the information

being published at some time if we see fit.

4 What you think your condition cost in 1999 (medical visits, medications

and therapies, lost earning time)
