Telehealth Survey

We are researchers at the Institute for Child Health Policy at the University of Florida.

We are working with the Florida Developmental Disabilities Council to

help improve services for people with developmental disabilities. We are

especially interested in hearing from you. We want to learn more about the

kinds of services people with developmental disabilities need. We also want to

learn more from people and families about their interest in a new way to

receive health care, called telehealth. Telehealth is a way to get medical service (doctors visits,

education, therapy, counseling or care coordination) by two-way TV,

videophones, or computers. If available you could “be seen” while

you are in a clinic near your home, or sometimes from home, while the doctor or

therapist could be in a university or clinic far from you. You could talk back

and forth and see each other during the visit.

What we are trying to find out is, what kind of services do people with

developmental disabilities routinely need? Could some of these services be

delivered through telehealth? Would people be satisfied and use this kind of

service? To answer these questions, we need your help. We need families to tell

us about the kinds of services they need, and their ability to get it.

We would like to ask you to complete a short on-line survey about

services for people with developmental disabilities. It will take about 20

minutes of your time. You can access the survey using the internet at:

https://www.phhp.ufl.edu/telehealth/FDDC/index.html

Participating in this online survey is completely voluntary. Your

participation is also completely anonymous! We will collect no identifying

information about you. No one will know you completed the survey. Your answers

will be combined with everyone else’s answers from across the state and

reported at a group level only.

If you do not have computer access to be able to conduct the survey,

you have several options. Most public libraries have internet connections

available. You can go there. Alternatively, you can mail the enclosed card

back to us and we will send you a copy of the survey for you to fill out and

return to us.

We appreciate your help! If you have any questions about this letter

or the survey, you may contact Dr. Lise Youngblade

at 1-, ext. 86269, or Charlene Seal

at the Florida Developmental Disabilities Council, 1-.

Thank you so much for your time!

Sincerely,

Lise M. Youngblade, Ph.D.

Associate Director, Institute for Child Health Policy
