Any thought about a team effort response? I went to the comment section and it

is pretty detailed. I’m willing to work w/others to craft a recommendation

and then post. This is important to get the RD name and understanding that a

grade C to not recommend counseling until an issue arises is the same issue we

have today. Preventative services are more cost effective in the long run- at

least that is how corporate wellness is being sold to companies w/health

insurance. The ROI for a company to offer a wellness program is essentially a

preventative approach.

I also noticed the grade C recommendation - it seems to be based on the

ineffectiveness of the minimal contact counseling but it also mentioned more

effective longer term contacts which the RD could be providing.

" Overall, the most important factor for differences in effect sizes was

intervention intensity. The strongest evidence for improvement of physiologic

outcomes was for high-intensity counseling interventions. Few medium-intensity

interventions and no high-intensity interventions involved primary care

clinicians as the provider of the intervention. Rather, counseling

interventions took place in other sectors of the health care system or

community settings. "

A Grade C is not a recommendation at all. A group effort to urge a Grade B at

least would be good.

C = " The USPSTF recommends against routinely providing the service. There may

be considerations that support providing the service in an individual patient.

There is at least moderate certainty that the net benefit is small.

Offer/provide this service only if other considerations support offering or

Last time I checked, healthcare.gov listed nutrition education as a grade B

recommendation. This means our services will be covered 100% if the Affordable

Care Act is left intact. The intent of the comment period is possibly to

determine who will be allowed to provide nutrition counseling - nurses,

doctors,

chiropractors, Weight Watchers, Craig, personal trainers, etc. We need to

let USPSTF know why the RD is uniquely poised to offer the service more

efficiently and effectively; it is a matter of resources. What I am able to

offer the patient newly diagnosed as diabetic in the acute care setting would

score a " C " but we have the opportunity to be independent providers of nutrition

education under ACA. Speak up now or live with the consequences.
