NCC - CINN and Dr. Heffez letter - hotels & testing

For those of you waiting for a response from Dr. Heffez, I wanted to

post a few details that I received in my letter that may be helpful

in answering some general questions that I had prior to receiving.

2-DAYS OF TESTING

(I have possible cervical stenosis/foramen magnum stenosis): FYI -

included in my two days of evaluation are: Physical Therapy

evaluation (rehab), Occupational Therapy Eval.(adapting to work &

life), Behavioral Medicine Eval (diagnosis concerns & coping with

illness), Speech-Language Eval (speaking & thinking skills), MRI, CT,

Dr. Heffez appointment to review all testing & discuss treatment plan.

HOTELS: within 1/2 mile of the hospital - these were included in my

letter:

1. Lakeshore Guest Unit (one of the floors of the hospital, revamped

forvisitors) 1- - $70 per day, no tax

2. Days Inn of Lincoln Park 1- $70-$90, plus 14.9% tax

per night

3. Comfort Inn 1- $58 - one bedroom, 1 or 2 people

$78-two bedroom, 1-4 people.

4. Belden-Stratford 1- or 1- $99 - deluxe

room $125-suite; $80 per night for 4 nights or more.

All rates are discounted for patients and family of Columbus Hospital

and the Chicago Institute of Neurosurgery and Neuroresearch. When

making a reservation, please mention that you are requesting the

Columbus Hospital Rate.
