Guest guest Posted February 28, 2011 Report Share Posted February 28, 2011 I think a perfect example is Oncology Dietitians. Too many centers and cancer groups do NOT have an RD on staff. I feel so many cancer patients are helped through the emotional and physical problems that can occur during treatment. I used to see all my breast cancer patients in radiation. So many just wanted to know what to do after treatment was over and they had the rest of their lives to live. Even if the cancer can not be detected they still want to live healthier lives. When I was looking into private practice work initially, scoping out the market, I learned that a lot of potential RD visits are not being billed/provided. Even within the current insurance billing every single person in the health plan that I spoke with could get one nutrition visit a year for general prevention services if referred by their doctor. And gastric bypass patients and a few other types of patients could get a lot more billable visits (that weren't being used). I think there could be a great market for a contractual floating dietitian, offering services within several physician offices providing each of them with a multi-disciplinary team without increased employee benefits. The easiest billing approach for nutrition services seems to be under the physician or chiropractor. They can buy into a variety of " Nutrition System XYZ " plans/product bundles and have anyone teach it and make bucks on the sales. Where is the incentive in referring to a dietitian? We should be as respected and utilized as physical therapy, occupational therapy, etc. So if RD's want to be the educators then we have to ask for the referrals and explain the difference between individualized nutrition science and cookie cutter nutrition education. Chiropractic offices are billing for nutrition sessions with a non-nutrition professional and it counts as long as it is on the same day as service was received with the chiropractor. Billing for nutrition education and counseling services is becoming a revenue stream that we need to protect - not just for our profession but also for the protection of the consumer. The information is not necessarily correct and when crammed onto an appointment for a different service will probably leave little time for quality improvement. Vajda, R.D.
