*Title of Study: *Front-of-Package Nutrition Symbols and Behavioral Intent**

You are invited to participate in this optional survey of two nutritional

scoring systems. I am a graduate student at the University of Connecticut,

and I am conducting this survey as part of my course work. I am interested

in finding out if there is a relationship with nutrition symbols and

behavioral intention. I have received approval from DBC DPG/ADA to

disseminate this survey.

Your participation in this study will require completion of an online

questionnaire. This should take approximately 20 minutes of your time. Your

participation will be anonymous and you will not be contacted again in the

future. If you wish, you may enter your email address into a drawing for a

chance at one of six $50 gift cards to Amazon.com.
