Breastfeeding Questionnaire - Help please

I am doing my thesis on breast-feeding and celiac disease and am having a

difficult time finding a valid and reliable questionnaire to answer the

questions on breast-feeding, including its duration, and exclusivity. If anyone

has any information that can help me it would be greatly appreciated!
