Guest guest Posted November 13, 2008 Report Share Posted November 13, 2008 I do inpt nutrition counseling at a large hospital and also work in an LTACH/rehab where we do provide extra Vit C/Zinc for wound healing. We provide evidence-based care and feel that this is successful for healing. However, the team would need to monitor for continued need of the supplements to prevent toxicity, etc. Also, if the would is healing where supplements could be tapered or if the owund has progressed where then supplementation would be continued. If severely nutrient depleted, based on risk/assessment, then a standard Multivit/mineral may take months to replete the patient, and thus wouldn't be close to adequate. If the patient has diarrhea, then a standard MVI/Mineral won't be adequate to replenish. Also depends on what/how the patient is eating/fed. . . If receiving only " canned " supplements without any antioxidants, needs vary from patient that's eating plenty of antioxidant containing fruits/veggies. What is protein intake based on needs? I've seen wound patients that required over 200 grams protein per day to heal. . . the dietitian that had preceded me had been charting, " Intake adequate for needs " despite ongoing multiple stage 4 pressure ulcers. .. the guy had been 6'6 " his entire life with a " healthy " weight of 250# (very large frame), but his height was recorded in the medical record as 5'10 " and his IBW was estimated at something like 170 so needs were very undercalculated. The facility didn't bother to do an adequate intake history on the guy. Also, were wounds staged? Treatment for stage 1 wouldn't be the same as for stage 4.
