Which of these symptoms does your LO have. Please mark all the are applicable.

Although not a symptom of Lewy Body, I'm including maculare deganeration/loss of

vision because it's the condition that we blamed so many other symptoms (balance

problems, depression and loss of interest in things etc.) on before we received

the diagnosis.

o Bowel AND urinary incontinance

o Excessive sleeping

o Unable to feed self

o Hallucinations

o Non-communicative.

o Paranoia

o Outbursts of anger

o Knows loved ones

o Unable to ambulate or walk without help

o Needs help with all ADL's (showering, toileting, dressing etc.)
