ATTENTION NGOs/CBOs/Trusts

State AIDS Control Society, Chandigarh invites project

proposals from interested Non-Governmental Organizations

(NGOs)/Community Based Organizations (CBOs)/Trusts, engaged in

community work for three years or more, for implementing Targeted

Intervention projects among the high risk groups in Union Territory

of Chandigarh.

The Proposals must include:

(a) Background & Justification; ( Goal & Objectives; © Approach

& Strategy; (d) Activities;

(e) Work plan; (f) Proposed Outputs; (g) Monitoring & evaluation;

(h) sustainability; (i) Personnel Requirements; (j) Budget required;

(k) Reporting & Evaluation (l) Assumption and risk

The applications signed by the NGO/CBO//Trusts head with stamp/seal

must reach the Project Director at the above mentioned address by

January 10, 2007 alongwith the following documents:

(a) Copy of registration certificate

( Copy of memorandum of association

© Annual report/work plan of the last three years

(d) Audited financial statements & balance sheet for the last

three years

(e) List of projects undertaken in last 3 years clearly showing

budget, geographical area & funding agency

(f) Report on the organizational structure of the NGO/CBO//Trusts

(g) List of staff & executive members of the NGO/CBO/Trusts

(h) Self Assessment Report of the NGO/CBO/Trusts

(i) Certificate for not having been blacklisted by any

government organization

(j) Certificate for not having received grant for the same

proposal
