Guest guest Posted May 8, 2001 Report Share Posted May 8, 2001 I guess I would like to review some uterine anatomy that may help all of us to understand what all of this means. First of all the uterine lining (endometrium) which sits on top of the smooth muscle layer (myometrium) is divided into two regions, the lowermost layer or the basalis and the overlying uppermost layer or the functionalis. There is no clear distinction between these two layers. The functionalis is shed during menstration and responds to all of the hormonal changes during our cycle whereas the basalis and the residual functionalis (after menstration) are responsible for regenerating the endometrium. The basalis should not be lost either through menstration or through curettage (the C in the D & C) because it is needed to regenerate the endometrium. When the butchers suction and scrape too vigorously the basalis is lost and the endometrium can't regenerate. As I said before, this distinction between functionalis and basalis is arbitrary and the thickness of each varies. So if enough of the basalis is left behind there is the chance of regeneration but if it is completely lost there is no way to get it back. HOpe this helps!
