I have been using the NCM since Jan 2011 when I started my own practice and I find it quite useful even when I have to translate most of the guidelines. What I've been saying to my clients and patients is what I truly believe; that I don't believe in " diets " , that I believe in eating the right foods that will help them be better nourished, and if ill, to heal ease the effects of their condition. I call the instructions guidelines for them to learn more and then discuss with me again those they can't follow so we can find alternatives. This has taken me out of the " mother " role and turned me into the colleague and mentor. Although in some of my settings I don't get to do frequent f.up visits, so far I've been very pleased with the outcomes, even those I was not expecting any progress with, and have even attracted family members to be present at the second home appointment. I didn't realize your responsibility in changing the set up and organization of the manual, so thanks for the important detail. Although diabetes has never been my favorite, the majority of my clients are diabetics. I have found the carb counting a tool of incredible value. I enjoy watching their faces when there are no forbidden foods and read the huge numbers of foods they're told they can eat. I see the outcomes if I do a f.up in the blood sugar levels (they all keep logs) and weights. In my opinion even 3 days with clear liquids is too much in patients with high risk of malnutrition. Several studies prove that even 24 h fasting with only clear liquids increase metabolic stress. Usually in those patients I allow the liquids that they can eat but use some kind of nutritional support as well, enteral or parenteral. Even though oral nutrition products don't taste that great for some people (although I have tried all of them and and most taste good)they are usually well tolerated if you use low osmolarity ones. With the oral nutrition support I talk with the patient to check their preferences if they don't like the taste of a product I usually change to the other brand. After GI surgery, if the patient is doing TPN I keep it until they can fulfil around 60% of their energy needs PO. If they aren't doing TPN instead of starting with clear liquids diet I start with an oral nutrition support product. Patients usually tolerate well. I always talk with the patient explaining that enteral nutrition is part of their treatment and will help them in recovery. Never had any of them rejecting the products.
