It sounds like its part psych, and possibly part physiological. If testosterone

levels are low normal this could cause an issue even though they may still be

WNL. Same with blood sugars, or even Vitamin D & B levels. Otherwise without

speaking to him and getting full stories not really sure. He may be used to a

much higher testosterone level and now that it's on the lower side it may be

throwing him " off " .

Subject: New onset ANGRY outbursts in a 60 y/o male

Any ideas about a 60 y/o guy that was never a nasty person.

(For example he made a huge scene yesterday at a grocery store because a

clerk sent him down the wrong aisle twice. Even it scared/surprised him in

retrospect.)

In the past year, he's had issues of extreme angry outbursts. (His abusive

father passed away in May of last year of brain cancer - but he wasn't

having angry outbursts then.)

Diet and exercise/lifestyle hasn't changed significantly except for getting

out of a bad 3 year relationship (but has often had relationships that

turn bad, but not due to anger issues).

Takes a good variety of supplements, B-vitamins, etc. Basic blood levels

normal. Only cholesterol very mildly elevated. Thyroid levels " standard

normal " but I think his testoserone levels are borderline low - oh, said his

libido has plummeted, but also dealing with breakup again.

Docs and psych have examined him and can't give him any ideas - said they

don't know what's up.

Even with drinking - he turns into a lovey, sleepy guy and goes to bed.

Has always been ADD and dyslexia - but never on meds for these. Does take

depression meds but never dx with bipolar and doesn't have the highs and

crashes of bi-polar.

Intelligent guy. Authority issues. Mostly works for himself most of his

life (or doesn't work). Grew up in a wealthy but highly abusive home.

He wants to know " Why am I NOW getting so angry over little things when

I've never been like this before? "

Anyway. This is what his SISTER has told me (she's a dear new friend - I

don't know her brother) because she speaks by phone often with her brother.

Sounds like hormone changes? Or exacerbated PTSD? He IS working with a

psychiatrist.

He's wondering IS this psych? (With such new onset) Or something more

physical related?

Specific ideas welcome.

I'd be looking for a brain tumor or SDH (subdural hemorrhage) if it's not PTSD.

Thanks everybody.

Just an update. He went back to his doc, maybe even got a new one - being

" in tears " the health care team he's worked with all agreed this " was not

him " - so they upped his Xanax. Sigh. . . I won't say more about that. (He

was on a minimal dose -they'll give him a few days and check response.)

His father died of a brain tumor in the past year, I think they ruled that

out for him. Ruled out early Alzheimers and dementia as best they could.

I think they need to dig further on the PTSD, depression (current

relationship meltdown+father's death in past year+caring for formerly severely

abusive mother with psych issues) and hormone levels. I also think low blood

sugar was R/O. Maybe chemical exposure as well - he's the " house cleaner " and

his sister is very chemical sensitive.

Full combo likely. Thanks again.
