I went to their email site and got this address.

Sent them a note and included the original email sent to Ra-Support. Hopefully

I will hear from them.

I went to the address, http://www.medquery inc.com/SelectSurvey -- then I was

prompted for the Survey ID which was listed with the info (SurveyID=70J78m1355

2KG) -- worked good for me! Hope this helps.
