Isn't the normal or average prescribed dose 1 mg a day with MTX? Guest guest Posted May 16, 2008 Report Share Posted May 16, 2008 yes there is, but i don't know regarding the difference in dosage. i suppose that's the difference...LOL, i just thougth 4mg a day seems to be too much. that's what made me ask about the OTC. trying to learn about it myself. I remember asking my rheumy though if we should increase me to 2 mg a day. He stated no, because too much can hinder the effectiveness of MTX. Guest guest Posted May 17, 2008 Report Share Posted May 17, 2008 Heidi, My folic acid is prescribed. I am taking 4 (1 mg) tablets per day. Maybe the folic acid dosage depends on the MTX dosage. I take 5 (25 mg) tablets once a week. I do know that several years ago my folic acid dosage was increased as a result of these little blistery-type things (very hard to describe) that I developed on my fingers (I think because of my high uric acid level). Increasing it took care of the problem. I should probably tell you that I also suffer from Gout, and the folic acid is part of the treatment for that as well as the RA. This is very  complicated and confusing.I did send a previous post you could probably find on the subject.... In Britain we would say 400 micrograms .That is just less tha 500 micrograms (the m you may notice is written with a little tail, it is a Greek small m which makes all the difference) This is a very small dose as given in vitamin supplements, supposed to equate with a good dietary intake and no more. 500 micrograms is just half a milligram. So usually about ten times the OTC dose is used in therapy with methotrexate by the doctor. The aim is as follows. When used with methotrexate folic acid should be prescribed by the doctor at a higher dose to hopefully counteract some but not all side effects of Methotrexate on healthy dividing cells of mouth gut, skin etc. (When they slow down how fast they replace themselves mouth ulcers etc can occur) Of course that is the way that the methotrexate is going to work on the overactive inflammatory cells so it is a risks/ benefits scenario and often a fine balance for each individual. Evidence is that the precise dose can be tailored to the patient, doses range from between 1mg daily, except day of MTX, or 5mg on day after MTX (this commonly used in UK) to higher doses. The 24 hour after taking MTX is usually kept clear of folic acid, so in theory the desired MTX effect on the immune system can be unimpaired. Folic acid 10mg daily or even higher have been used on other days of the week,and although this would be uncommon in uncomplicated rheumatoid.it is perfectly safe.
