We often make the mistake of trying to take what we have on paper and translate that exactly to a clinical system. That never works well. Instead, think of the questions on the MST. Both are most likely already asked by nursing on admission; if not, add them to your nursing admission screens. Then the results can be sent to you as a report or however you want to see them.
