I have to keep a record of my outpatient education sessions for the hospital

records so I use that, then I also document on any inpatient education I do. I

was also told no particular format, and that they really don’t check into them

much, I’m not sure how true that is though.

I am in the same position, planning to take exam in November but do not know

much about it. The stuff on medications seems like it will be hardest. How are

you all documenting your DSME hours. I am going back to some old jobs to use a

portion of what I did there and then will have my supervisor sign. I am also

working one day a week at a diabetes clinic to get hours and will have the total

signed off on. What do you think? I was told by the organization there was no

particular format, just have it documented.

Please let me know how you know others have done it or how you plan to do it.

I'd start there first. If she has a problem with the soy, Bluebonnet sells a

tablet without the soy. Also, magnesium is vitamin D's biggest cofactor...... I

would also add some magnesium (not oxide) to her supplementation regimen to make
