We have a community health center yet I don't think they are much of a help.  In

our state, most are sent to the rehabilitation centers for assistance.  Luckily,

with my panic disorder, I am able to do most things for myself..yet because of

other health issues and cognitive dysfunction I have appointed my mom as a power

of attorney for when I cannot.  I'm sure the link Jolene provided will have very

helpful information with advocates that can help
