Guest guest Posted October 14, 2011 Report Share Posted October 14, 2011 I worked once where a physician was quite thorough in his history and would include where the patient's parents attended school and how far they went. (The patient's were in their 80's, so these parents were long dead and attended school at the turn of the century--this was 25 year ago). One of the funniest things I think I ever saw was not in the medical record, but a note taped to the front of the chart. It read, " If Mrs. does not eat, call her nephew, Bill , and I will eat for her. I am staying at such and such Motel.
