contact information to the manufacturer associated with the survey for any

consent to the release of your contact information for these purposes if one of

your responses suggests the occurrence of an adverse event.

Epocrates Honors Visa debit card, which can be used in millions of Visa

locations worldwide. If you already have an Epocrates Honors Visa card, your

honorarium will be credited to your card 3-4 weeks after the survey close date.

For new participants, please allow 4-5 weeks for delivery of your Epocrates

Honors Visa card and honorarium. Please do not destroy this card, as additional

honoraria will be credited to this card when you complete future studies.

> We encourage you to be an active Epocrates participant and to use your prepaid

card. Please be aware that to the extent permitted by law, your card has an

expiration date, at which time inactive accounts will be terminated. Your card

must be activated prior to its initial use. If you do not activate the card

within 12 months, all funds on the card will be forfeited. In addition, a

service fee of $2.50 per month will be charged against outstanding balances on

your card (to the extent permitted by law) after the account has been inactive

for 12 months. For these purposes, an account is considered inactive if no new

funds have been loaded (as a result of completing new surveys), the account

balance has not been checked through the online service or no funds have been

spent from the account during this period. Once any of these activities occurs,

the account will once more be classified as active.
