Dear VICTORIA WEAVER,

This communication alerts you to an upcoming marketing research study

of interest to your profession.  This is not a solicitation or

advertisement to sell you any product or service.

We are looking to gain feedback from a selected group of dieticians,

like you, to discuss Dialysis Nutrition.  The telephone interview

should take approximately 45-60 minutes for you to complete.

For your time and opinions, we would like to present you with a

CASH INCENTIVE OF ($200).

  You must qualify and then complete this interview to receive this

honorarium. Please allow 4-6 weeks for processing after the project

closes. If after this timeframe you have any questions regarding your

If you qualify, you will be called to schedule an appointment for your

interview within a few days of filling out the online screener. This

online portion is only the screener and you will only be paid the

honoraria listed above once the telephone portion is complete.  There

is no incentive for the screener.
