The problem with current TF plus oral supplementation his BUN are over 100 and doc wants him to decrease prot intake. Pt wants to gain weight and is fearful with decreasing TF he will lose wt. Does anyone have any recipes or suggestions on high Kcal low prot foods-- he can not tolerate butter or a lot of high fat items. Thank You. Guest guest Posted April 3, 2012 Report Share Posted April 3, 2012 Is the patient already doing pancreatic enzymes, usually cystic fibrosis patients have an impairment of exocrine and sometimes endocrine pancreatic function. The supplementation with those can improve the tolerance to fat. You can also supplement with maltodextrines added to the fluid that he is allowed to drink and add jam and jelly to bread and deserts. If the patient has good dialysis efficacy why the doctor wants to decrease BUN? How is his Kt/v and URR? Does he have enough vitamins and minerals to optimize protein metabolism and increase of body weight by muscle mass? I would also recommend the patient to start resistance exercises to promote increase of muscle mass (walking isn't enough in these cases).
