It's almost a month and nobody in the forum so far has responded to his query. (I am not sure if anybody has written in his Private email id). Many of us must be practising clinicians or people who are strong in ART. Here is one person running a helpline and I believe he must be getting lot of calls from people are worried that may be they are resistent to the first line drugs and wants to know what is there as second line. Since, we cannot wait for NACO probably some of us would like to start by ourselves. If at all we have to start we would like to know how much will it cost and so on. We have been seeing so many intellectuals writing about second line and all in this forum. Many analytical and critical articles or isuues are being brought up. Can somebody also enlighten all of us on this through the forum? Ideally second line drug should be selected with an available viral resistance study-which is very expensive a difficult to get done in India. Some body who was on standard WHO/NACO first line can be put on Tenofovir /Abacavir +DDI +Retonavir boosted lopinavir or saquinavir. Remember all these drugs are costly and associated with side effects and need close supervision. Most of the second line drugs are available in India now Probably no doctors tried to respond to this query because it is too complicated question to be answered through a simple mail and before providing response through help line it will be better to contact a doctor who is well worse in second line therapy.
