swing bed

Hi,

I work at a critical access hospital and new to this type of setting.

We have a swing bed unit and the staff here stated that OT and speech

are unable to qualify patients-only PT. Any insights?

I have a swing bed question for the group. The Swing Bed MDS Training

Manual under Ordered Therapies (page 2-76), states.... " If orders exist,

consult with the therapists involved to determine if the initial

evaluation is completed and therapy treatment (s) has been scheduled. "

My questions are related to the patient whose status changes from acute

care to swing bed -- within the same facility. My example: if a PT

does an acute care eval on Tuesday and the patient's status changes to

swing bed on Wednesday -- does that therapist need to do another eval on

Wednesday, or can we use Tuesday's acute care eval for the swing bed

stay??? Does " initial eval " refer to, or include the acute care eval?

We have always done a new swing bed eval, but are now questioning if we

really need to. I'm hoping someone can help shed some light on this

question. Thanks in advance! Laurie

We keep the same eval across the board. In Rehab, we start a new billing

grid. Nursing does some minimal change over. The chart is stamped

" Transfer Acute to Swing Bed " or something on all continuing pages and a new

page is started (physician notes, nursing notes, etc.) We set a policy to

that effect. Our Health Records Department Head at the time coordinated

with the State Health Department to ensure our compliance. Health Records

really worked out all the details. Makes sense. More efficient. Invisible

to the patient. No concerns with physicians.

I believe you still need to do an evaluation. We have a swing bed eval

that is a bit shorter that the original in this case, but it still

reassess and sets new goals. Seems somewhat wasteful but I believe you

need to view this pt as if they came from another hospital.

This occurs in our facility too. The patient may even stay in the same bed but

their status has changed. When a patient moves to a Swing bed status

(considered SNF) level of care, a re-eval must be done. The med surg medical

record is 'closed out' and a 'swing bed' medical record is created. Our

treatment goals for med surg patients are often very different that goals we may

develop for a swing bed patient. I know it is a lot of paperwork... we struggle

with that also.
