Since, technically, his Rx for foot pain is expired, we would need a new referral to continue treating the foot. Regarding billing, because we provide PT and OT, he would have 2 charts (1 for each condition). If a PT is treating for both conditions, you would have 1 chart. You would only be able to bill another Initial Evaluation if the CPT code is different from the one initially charged. Otherwise, it would be a Re-evaluation.
