Employee rules and regulations

I was wondering if someone would be willing to share or direct toward a

resource to help me develop an employee handbbok of rules and regs for

an outpatient rehab practice. I am also looking for an incident report

suitable for outpatient use but have had difficulty finding one. I

would appreciate any help
