Group: I was reviewing this draft LCD again and noticed it states do not record

time in and time out to justify your treatment. At our facility we do time in

and time out; time per each CPT code (i.e. Eval: 30 minutes, Ther. Ex: 20

minutest, etc...); and total treatment time (i.e. 50 minutes). Is documenting

time in and time out an over kill to meet the LCD requirements?

We record time the same way you are now. I believe BCBS or another private

insurance requires time in/time out.
