Group,

It's been 3 years since we brought this up, long enough for new and

enlightened opinions. Do you have AED's in your free standing PT clinics? Do

you get advice from local EMS crews regarding the need in your settings.

Have any clinics who have them, actually used it?

Any and all feedback greatly appreciated!

Jeff,

At Henry Ford Health Systems in Detroit, all our outpatient clinics have an

AED either in the clinic or in their building, if that building also houses

additional medical services. When EMTALA laws became effective several

years ago, I believe that EMTALA stipulates that we should be able to

provide these services, consistent with all available services in our

organization. I am by no means a legal scholar when it comes to EMTALA law,

however since you are located in an inpatient setting, I suggest you inquire

with your legal and risk management department.

We have been fortunate enough that to date we have not had to use them. All

our staff is CPR certified under the American Heart Association's standards

for health care providers, which include training in the use of AEDs.

We have one in our clinic and we are sub attached to a hospital.

However we are too far for a code blue or crash cart to get here in a

timely manner, we do not have ambulances on standby all the time either.

So we bought an AED, brand. One of our ortho docs says it's

not if but when a patient will go down with a post op embolism, and even

though an AED might not save them we want to be able to do everything we

can if a patient doesn't have a heart beat. Ours also monitors EKG info

while it is on.

We do have AED's in our free standing clinics. We feel that having them

is a no-brainer. We would rather have one and never need it than to not

have one and need it. It would be well worth the expense if used even

once.

Also, with AED's becoming so common place these days; if you did need

one and didn't have one and the person that needed it ended up passing

away it could be a situation where your clinic could be held in question

as to whether or not you had made reasonable efforts to prepare for a

cardiac emergency. Considering the medical nature of our business, the

often high utilization by geriatric individuals and the litigousness of

our society I think this could make for a bad situation for a clinic

without an AED.

My two cents.

Although not required we have them. If the local mall has one kind of

difficult to explain why we would not. All staff also have AMA BLS.

Never used, thank you.

Jeff--I own a small clinic in a small town, and we have had an AED for about 2

years. The local emergency services chief has been over to our clinic, and since

it is one of only 4 in town, we get a plug every time he lists the units within

the town at a meeting...We have never, thank god, had to use it, though I have

brought it into a treatment room while waiting for an EMT squad to arrive for a

patient, and that helped make the purchase worthwhile...It is worth an internet

search for the best price, even the low end ranges about $500 between low and

high price for the same unit....Jeff Woodruff, Woodruff PT, Chester, NH
