chronic Achilles tendonosis

I am looking for advice on treatment for chronic Achilles tendonosis. I have

been treating a patient with the above Dx. Initially I did ASTYM for 8 visits

with good results and 80% subjective improvement. After a 4 week layoff his

symptoms and swelling returned. I have since referred him for orthotics which

has helped and have resumed ASTYM. I am trying to convince him to consult with

the referring podiatrist but this patient is adamant about refusing injection

therapy and is questioning my suggestion of Iontophoresis or Phonophoresis.

Any suggestions will be greatly appreciated.
