On OP, we also write daily contact notes ( e.g 2x or 3 x a week, depending on number of visits that week). We also write a monthly summary. Guest guest Posted November 19, 2007 Report Share Posted November 19, 2007 Bob - The following is from the Medicare Benefit Policy Manual (Chapter 15): " The Progress Report provides justification for the medical necessity of treatment. " ~ and ~ " Timing. The minimum Progress Report Period shall be at least once every 10 treatment days or at least once during each certification interval, whichever is less. The beginning of the first reporting period is the first day of the episode of treatment regardless of whether the service provided on that day is an evaluation, re-evaluation or treatment. Regardless of the date on which the report is actually written (and dated), the end of the Progress Report Period is either a date chosen by the clinician, the 10th treatment day, or the last day of the certification interval, whichever is shorter. " It 2008, based upon the new regulations, the Progress Report is now due after 10 visits, or at least once during the certification period, (90 days), whichever is less. The 30 days you referred to in your post are related to the certification period, rather than a 30 day requirement on the Progress note itself. If the Progress Note is utilized as an Updated Plan of Care (all required elements included), then the 30 days apply, unless the 10 visits were achieved prior to the 30 days (the certification period in 2007) The Progress Note in 2007 is 10 visits, or at least once during the certification period, which is 30 days, except for CORFs, which in 2007 were 60 days. The therapist can always elect to write additional progress reports as necessary. This may be clarified in expected transmittal(s) at the beginning of the year. J. However, they currently require progress notes every 10 visits or 30 days (which ever is sooner). Is there any indication that the " 10-visit " rule is going away or changing? A progress note is completed at 30 days and 60 days per Medicare requirement of progress note 10 visits or 30 days, whichever is first. Now you have a POC due at 90 days if the patient is continuing therapy. Does the updated POC take the place of the progress note at 90 days or are both required at 90 days? Thanks for your input. At the top of the form are boxes that indicate that the form fills multiple functions : 10 vitis/monthly summary/ recertification/ dicharge.
