Do you believe that doing a Home Evaluation is best practice and should it be a standard of care for patients in an Inpatient Rehab Facility? If so, Are there any IRFs that are consistently completing a Home Evaluation for a majority of their patients with discharge plans to go home? At what point during the patient's stay do you complete the home eval? Who does the home eval? Does the patient go on the home eval or just the therapist and the family?
