Dear colleagues,

I am currently reviewing acute care practice trends with regards to the

delivery of physical and occupational therapy to identify best practices

and improve appropriate access to care for our patients. For those that

work in the acute care setting, please click on the provided link below

(if the link does not work, please copy and paste it into your web

browser's address bar) and complete the attached survey at your

convenience. The survey should take 5-10 minutes to complete. All

respondents will have access to the data at the end of the survey if so

desired, and a follow up report will be sent via e-mail. To ensure

confidentiality, no data containing facility names or locations will be
