The question was recently asked of whether all notes had to be physically signed by the therapist since their name appears on the printed document and since there is an audit trail and way of knowing who indeed was logged onto the system when the note was completed. I know I have always been taught that it is not official unless it is signed. I know the answer is likely that the signature is still required, but what official document can I reference to validate that this must be done? Your input will be appreciated.
