> The EMS Conference and subcommittee meetings are the perfect stage for us

> to allow you to provide us with your contact information in order to

> initiate setting up your EMS/Trauma Registry account. There will be

> several laptop computers available for you to enter your information that

> will be placed into the new system. Our staff will be there to assist you

> and answer any questions you may have related to the new EMS/Trauma

> Registry System.

> The information that you provide will be used to start the process in

> setting up your EMS/Trauma Registry account.

> You may pick one of these options:

> Manual Entry of your entity contact information at the Conference Booth:

> A letter will be sent, post conference, from our office requesting that a

> letter (on official letterhead) be sent to our office identifying the

> person who will have administrative authority over your EMS, hospital or

> RAC data account.

> OR

>

> Complete Attached Data Form:

> You may complete the information in the attached access data form and save

> to a disk (3.5 floppy) and give it to EMS/Trauma Registry program staff at

> the conference booth.

> OR

>

> Complete the data form information, save on 3.5 floppy disk and mail to

> our office with a letter (on official letterhead) identifying the person

> who will have administrative authority over your EMS, hospital or RAC data

> account.

> Mail via registered mail to:

> EMS/Trauma Registry Program

> It is our hope that this will assist the transition to the new Texas
