Social Security reviews your documentation that your provide and the checklist

has the required documentation. If you go down to the office, the

representative will ask the questions and type them in as you are sitting there.

Anything they ask as a request, there is sometimes another sentence that says,

Letters from co workers, friends, family optional. Do this, it will sound like

you don't need it but it helps with your case !

That is why I say don't let them " send you the forms " go down to the office and

fill it out there from the checklist, that is one of the options and the

representatives are helpful.

Tami must have been denied the first time as she had to get a lawyer and I was

not denied the first time as the representative said " Wow, you are prepared "

Even though I had my Doctor's documentation I did have to go to the Social

Security designated physicians and a psychiatrist since I marked I had

depression.

These are just regular Doctors and they don't get a bonus or commission if they

deny your form. Your determination is based on documentation of what you have

said you are requesting disability. I told all my Doctors I was applying for

disability and would they support it, and their answer was " We were just

waiting on you to say " When " .

I really think you can win the first time if you do this. Please look at the

SSI disability site and look through everything. I am not saying Tami is wrong,

and also, if you do get a denial, SS states they will provide you a list of

advisers (not the word they use) that will assist you with the fee no cost.

The main thing they are looking for is that you cannot complete job tasks due to

your diagnosed disabilities. I have a functional assessment form my Doctor had

me use so he could see. You think things like cannot cook for my family, clean

the house, drive on trips, complete my job tasks within my required job

description due to the...Depression, degenerative disc disease, failed back

surgery, SI syndrome, fibromylagia. I am unable to drive due to the medications

I have to take for my conditions and present safety issues. My family does the

jobs I normally did...list the things, unable to sit over one hour, stand for

one hour.

This is the most important area that people do not list as they think it

unimportant. If I can assist you so you do not have to pay lawyer fees, I will

be happy to.''

An example is muscoskeltal and that is what I clicked an it says that you must

be able to document the impairment, pain, and inability to ambulate and perform

normal functions you did before

Further they state documentation includes:

General. Diagnosis and evaluation of musculoskeletal impairments should be

supported, as applicable, by detailed descriptions of the joints, including

ranges of motion, condition of the musculature (e.g., weakness, atrophy),

sensory or reflex changes, circulatory deficits, and laboratory findings,

including findings on x-ray or other appropriate medically acceptable imaging.

Medically acceptable imaging includes, but is not limited to, x-ray imaging,

computerized axial tomography (CAT scan) or magnetic resonance imaging (MRI),

with or without contrast material, myelography, and radionuclear bone scans.

" Appropriate " means that the technique used is the proper one to

• Names and dates of medical tests you have had and who sent you for them.

• Types of jobs and dates you worked for your last 5 jobs. I had copy of my job

descriptions and listed each one I could not do which wasall

There is a disability planner online. So you can see if you take the time to

start a notebook and have you documentation, I think you will win you case.

That is what the lawyer will go in and do and once you have this notebook, every

time you have a test, ask for a copy of it, you can get a CD for you x-rays, Cts

and thing radiological for free.

FUNCTIONAL PAIN ASSESSMENT: How is the patient disabled from everyday work &

life?

The Oswestry Disability Index (2) and the Roland- Disability Questionnaire

(22) are hands-down the most commonly used and recommend outcome measure tools

used for assessing the disabling effects of lumbar spinal disorders. (28,34,53)

The Stanford Score is an attempt to combine the level of the patients functional

disability and subjective disability.

These questionnaires and dermotome maps (shows the areas of the body that are

effected for each spine level in this example) on the link believe that is

Once you know what they want from a disability assessment, you can prepare your

case yourself so it is worth your while to research the questionnaires.

Bennie (EVERYONE) Thank you so much for this detailed info. I am copying these

links and info and printing them. I think if I can get up to it I will be

focusing on this tonight and see what happens? I truly appreciate every one of

If I did not say it, I want you to know this came from my own confusion and

having to check everything so please don't think I knew everything, I asked

questions over and over and checked it out. I also had the same frustrations

and confusion on the Medicare Insurance program that followed twenty-three

months later so read every question and you might have to wait awhile if you go

to the disability office but I think it is the best way to start the process as

there is someone right there that you can ask questions and they ask you. That

is the one thing I think is most important and I have gotten the best

information by doing this each time.
