1. Probably rotavirus.

2. Not sure ... maybe Ziac (hypertension) or Zyvox (antibiotic)?

3. Probably Zovirax.

4. Possibly Cortisporin.

Hi,

I have four SLs and drug names that I need help with, please!

Thank you in advance. I am so thankful for you all.

PAST SURGICAL HISTORY: Negative. She was hospitalized for two days for

SL " rotor virus " at age four.

_______________________________________________________________

Mrs. ____ is a 58-year-old woman who comes to see me for follow-up of her

diabetes.

Patient has done very well. She is currently on Glyburide and Glucophage.

She is off of SL " Zylect " and Avandia.

________________________________________________________________

A: 1) Urinary frequency, concerned of a bladder infection.

2) Perioral zoster.

P: 1) SL " Servax or Serbax " ointment (cream) apply to affected area t.i.d. x

7 days, 3 grams;1 refill.

______________________________________________________________________

SL " Cleosporin " otic drops, 2 drops to the right ear t.i.d. x 7 days.

We discussed for patient to stop SL " Fenadrine " use for now. (This is an OTC

weight

loss supplement).

MUSCULOSKELETAL: Normal chewing. Normal swallowing. No SL " seprof " noted.

I found Nordette. Could this be it?

ephedra? I know that is an ingredient in many OTC weight loss supplements.

step-off, meaning a gap between bone fracture fragments. Is this an injury?
