The American Pain Foundation ,http://www.painfoundation.org/learn, then click on the Target Chronic Pain Notebook and it will list templates. In this section, Learn About Your Pain and then Publication, you will learn about Pain Patient Bill of RIghts, Resources, Resources specifically for military,veterans, target your pain, other information sheets that help so very much and current research. I have been with Chronic Pain Groups and The American Pain Foundation for years. The American Pain Foundation has done and ex tremendous job in the area of legislature and does a Pain Community Mail-out. Every organization meets a need of some kind. The Target Pain header will list separate sections and is truly a good tool. If you click on the Target Pain Section, it will lead you to the pain notebook, and other pages that has descriptors. Target Chronic Pain Notebook : I put mine in a three ring binder and printed more when I needed them and other information. The Pain Notebook is an easy-to-use tool to help you maintain a record of your pain and communicate your experiences. It's an extremely useful tool when discussing levels of pain, response to treatment and improvements in functioning and side effects.
