Suffice to say that I was not meaning to malign any cast techs with my comments. In fact we had one awesome one at the Shriner's who I think could learn the Ponseti method very easily (and another bozo who shall remain nameless and skill-less). So I am sorry, Beth, if you felt maligned and know that I respect the work that you do. I would be interested to know if you find that the cast applied after the tenotomy is a particularly difficult or sensitive one to do. I guess it depends on the skill level of the individual, and not whether they are a doctor or not. I know Dr. Ponseti did prefer to have doctors do the manipulation and casting, although maybe his feelings have changed on this matter. I'll ask him when I see him! (very excited!) -. My Grandma just died about 1 month ago, she was 103 1/2 years old and she only wanted to go home:) I did'nt mean to sound so sensitive I just want families to be sure they have quality people treating their babies and I think more and more there are going to be " Physician extender " type medical staff who will be a big part of treatment programs. Another thing families should watch out for is that the practitioner MUST be sensitive to the babies and not forcefull. When I mention the casting I of course do the manipulations first and this and the cast application should not be done with a fussy uncomfortable baby. This takes some doing some days and the process therefore can take time. As far as the cast after the Tenotomy, The Dr. should be the only one doing this procedure and it really needs to be one who is experienced and confident as there are some important structures that cannot be interupted. I am assisting in all of these and we do the cast together and it is no more important then the other casts, so we make sure the positioning is correct and the difference in this one is that the foot is put in 10 - 15 degrees dorsiflexion (foot bent up at the ankle) and the foot should be 70 degrees externaly rotated (foot turned out) as it will be in the brace. I will be interested in Dr.Ponseti's comments I hope to go visit again when other Dr.'s go.
