1. Has your child cultured Pseudomonas Aeruginosa (P.A.)? (If no, just send

that answer, and there is no need to go further.) yes

2. How old was your child when P.A. was first cultured? 3 years and 1 month

3. What hospital/clinic follows your child? East TN Children's

4. Where is it located? Knoxville, TN

5. Who is your child's primary CF doctor? Dr. J.

6. What treatment SPECIFICALLY for Pseudo was initiated when it was first

cultured? (Plese skip all of the other medicines or treatments which your

child does regularly.) TOBI 28 days on and off indefinately (he was

already on Pulmozyme or that, too, would have been added. Also, he might

have had a tune up as well but he had just had one 2 months prior)

7. After P.A. was first cultured, and treatment was initiated, how long

until it was cultured again? it has only showed up once since; that was about

6 months after the first one during an off TOBI month (Maltophilia is more

regular for him)

cultured? (Plese skip all of the other medicines or treatments which your

child does regularly.) TOBI on/off 28 day cycles and added Pulmozyme

7. After P.A. was first cultured, and treatment was initiated, how long

until it was cultured again? PA cultured in every subsequent culture, except

one or two cultures done a few hours after TOBI treatments (those were normal

flora)
