This is indeed a boon for people living with HIV and who are accessing 2nd line treatment. I take this opportunity to congratulate NACO and the team who had put the efforts in making this a reality in India. But also there are other issues like: (1) The people who were put on 2 drugs combination instead of 3 drugs by private doctors and who are on 2nd line but with great difficulties bearing the expenses of the same. They are deprived of the free 2nd line treatment just because they were treated by private doctors who did not refer them to Govt. hospitals or who did not had any information on the same. What about them? (2) There are other hospitals in Mumbai, still, other than JJ who are referring people to MSF and who are accessing 2nd line treatment from there. What about them???? Will they be now covered under JJ and its refferal units for ART (other ART Centers). (3) Regarding the other tests including viral load which is very expensive as of now, who will bear the cost of them as of now, will the existing ART centers or the pts. will themselves have to bear this. I am sure you will clarify this soon. As a person living with HIV, I feel proud to be in India and see that 2nd line roll out programme is implemented. The viral load test will be provided free after evaluation by a panel of experts at these centres. The panel of experts will be constituted at each of these 10 centres and will be known as SACEP(Stae AIDS Clinical Expert Panel). This panel will evaluate all patients refered from ART centres for conforming whether they fulfill criteria of clinical or immunological failure and will than undergo free viral load at one these 10 centres.
