Can some one help me how to do a good documentation for home health care from

the Rac review point.Any sample copy of evaluation, care plan and revisits

Alos the different test used for gait analysis

The Home Health Section of the APTA has a wealth of information that may be

helpful.

The " Partner " Program of the Section allows organizations and individuals

who are not eligible to join to also benefit from the membership. The

Section website store has several recordings related to HHPT documentation. At

the website you'll also find a self-study material named " Make Sure You

Measure Up " which contains info on objective tests useful in the HH setting,

including gait analysis.

Rick Gawenda's website also has recordings available on the topic of PT

The documentation standards expected by Medicare for home health are

basically the same as for other settings, and it needs to clearly demonstrate

Medical Necessity (medically reasonable & necessary standard), and " Skilled "

Services provided. If the PT forms for Eval, POC and re-visits cover these 2

main criteria they should be off to a good start.

I'll defer to others re: direct experience w/ RAC reviews, as I have none.
