Guest guest Posted January 24, 2011 Report Share Posted January 24, 2011 The Home Health Section of the APTA has a wealth of information that may be helpful. _http://www.homehealthsection.org/index.cfm_ (http://www.homehealthsection.org/index.cfm) The " Partner " Program of the Section allows organizations and individuals who are not eligible to join to also benefit from the membership. The Section website store has several recordings related to HHPT documentation. At the website you'll also find a self-study material named " Make Sure You Measure Up " which contains info on objective tests useful in the HH setting, including gait analysis. Rick Gawenda's website also has recordings available on the topic of PT documentation that is worth investigating. _http://www.gawendaseminars.com/_ (http://www.gawendaseminars.com/) The documentation standards expected by Medicare for home health are basically the same as for other settings, and it needs to clearly demonstrate Medical Necessity (medically reasonable & necessary standard), and " Skilled " Services provided. If the PT forms for Eval, POC and re-visits cover these 2 main criteria they should be off to a good start. I'll defer to others re: direct experience w/ RAC reviews, as I have none.
