There are also postion papers on the ASHA website that address telepractice. I am a part of a Medicare Admin. Contractor group; we have a monthly teleconference w people form most states monthly. I will bring this up at next Tuesday's call. Guest guest Posted April 1, 2011 Report Share Posted April 1, 2011 * In addition to the information already provided here are additional comments: Since this individual prefaced the statement with the fact that it is a non-Medicare patient, the best answer would be provided by the payer source for this individual. If the payer source does not agree or permit CPT charges that are non-face to face. Perhaps they would consider a carve out statement to their present contract stating a fee and conditions for how it will be covered. If not that, do they allow something like an ABN so that the clinician can negotiate a private pay fee with the patient. The web link below is a Medicare transmittal from 2005, stating background and policy of listed Medicare Telehealth Services. Although speech is not mentioned the guideline document may at least help the clinician decide how they would like to approach the payer source. http://www.cms.gov/transmittals/downloads/R43BP.pdf Transmital 790 from Dec 2005 shows the changes made to the Medicare Claims processing manual.
